Hair Transplant Side Effects and Risks: What Is Normal and What Is Not

Common, short-lived side effects after a hair transplant, the less common complications, and how choosing the right team lowers risk.

A hair transplant is a surgical procedure. Most side effects are mild and settle within days to weeks, but complications do happen, and the risk is higher when surgery is done without proper medical oversight.

This page is general information. Your own doctor should assess your personal risks.

Common and usually temporary

  • Swelling of the forehead, sometimes reaching the eyes, around days 2 to 5.
  • Pain or tightness in the donor area, especially after FUT.
  • Scabbing and redness in the recipient area for one to two weeks; redness can last longer.
  • Numbness in the donor or recipient area, which usually improves over weeks to months.
  • Itching as the scalp heals.
  • Shedding of transplanted hair between weeks 2 and 8, and sometimes temporary shock loss of nearby existing hair.
  • Folliculitis: small pimples as new hairs grow through.

Less common complications

  • Infection: uncommon when sterile technique and aftercare are followed. Spreading redness, pus, fever or increasing pain need prompt review.
  • Poor growth or low density, which can relate to graft handling, planning or patient factors.
  • Unnatural hairline from poor design, wrong angles or placing multi-hair grafts at the front.
  • Donor over-harvesting: visible thinning or a "moth-eaten" look at the back after aggressive FUE.
  • Visible scarring: a wide FUT scar or noticeable FUE dot scars with very short haircuts.
  • Reactions to anaesthetic or medicines.

How to lower your risk

  1. Choose a named, qualified operating doctor. The NMC has advised that hair transplants should preferably be done by doctors with formal surgical training. See how to check credentials.
  2. Share your full medical history and medicines, including blood thinners and supplements.
  3. Be cautious with very large single sessions, especially if you are young.
  4. Follow aftercare instructions exactly, and attend follow-ups.
  5. Keep your clinic's emergency number and know who to call after hours.

If you are unhappy with a result, wait until about 12 months before judging it, then get an independent opinion before any repair surgery.

Frequently asked questions

Are hair transplants safe?

When performed by a qualified, experienced team with proper sterile technique, hair transplants usually have mild, temporary side effects. Complications such as infection, poor growth or donor over-harvesting are more likely when surgery is done without proper medical oversight.

How long does swelling last after a hair transplant?

Forehead swelling commonly appears around day 2 to 3 and usually settles by day 5 to 7.
